What is the Eczema Area and Severity Index?
The EASI score, short for Eczema Area and Severity Index, is an eczema severity score used for the clinical assessment of atopic dermatitis.
It assesses the spread and severity of the skin lesion. The scale ranges from 0 to 72, with a score of 0 describing an inconspicuous or mild exanthema and a score of 72 corresponding to severe disease. You can use the EASI score to track the disease's progression or assess the therapy's effectiveness.

What is atopic dermatitis?
Atopic dermatitis is one of the most common skin diseases. It is chronically inflammatory and triggered by various factors. The disease often occurs in childhood but improves with puberty. It is often associated with bronchial asthma.
Typical symptoms of atopic dermatitis are:
- Severe itching;
- Dry skin;
- Scaly skin; and
- Eczema.
Criteria of the EASI score for atopic dermatitis
As the name Eczema Area and Severity Index implies, the EASI score combines the skin lesion's area and severity scores.
The area score indicates what percentage of a particular area is affected by eczema. It divides the body surface into:
- Head and neck;
- Trunk;
- Upper limbs; and
- Lower limbs.
The eczema severity score, on the other hand, is made up of various factors. It takes into account the intensity of:
- Redness;
- Thickness (swelling);
- Scratching; and
- Lichenification.
Area score
To determine the area score, you simply have to estimate the percentage of the body part affected by atopic dermatitis.
Percentage of affected skin | Area score |
---|---|
0% | 0 |
1%-9% | 1 |
10%-29% | 2 |
30%-49% | 3 |
50%-69% | 4 |
70%-89% | 5 |
90%-100% | 6 |
Eczema severity score
To determine the severity score, add together the intensity of redness, thickness, scratching, and lichenification.
Intensity | Score |
---|---|
None | 0 |
Mild | 1 |
Moderate | 2 |
Severe | 3 |
We have created a small graphic to help you better assess the factors of redness, thickness, scratching, and lichenification. Feel free to use it as a guideline:

How to calculate the EASI score
The EASI score was made up of the individual scores of the respective body region:
EASI Score = Head and neck score + Trunk score + Upper limbs score + Lower limbs score
Then again, the score of each body region is the result of the area score multiplied by the severity score and a corresponding factor. The factor may vary depending on whether the patient is over 8 years old or not.
Region score = Area score × Severity Index × Region Factor
Here is an overview of the specific factors to multiply by:
Body region | < 8 years old | ≥ 8 years old |
---|---|---|
Head and neck | 0.2 | 0.1 |
Trunk | 0.3 | 0.3 |
Upper limbs | 0.2 | 0.2 |
Lower lImbs | 0.3 | 0.4 |
How to use the EASI score calculator
This calculator is very easy to use. You just need to select the parameters you want to use; there is no need to enter any numbers. Just leave the calculation to us!
For your convenience, we have created the following step-by-step instructions:
-
Age — Please select the appropriate box and indicate whether this is a person under 8 years of age or a person who is at least 8 years old.
-
Head and neck — Go ahead to the Head and neck section.
-
Area score — Select the percentage of the area affected by eczema.
-
Redness — Indicate the intensity of redness.
-
Thickness — Select the severity of the accompanied swelling.
-
Scratching — Are there any scratch marks on the patient? Indicate the amount of scratching.
-
Lichenification — Please select the intensity of lichenification of the eczema.
-
Next section — continue the same procedure with the remaining sections trunk, upper limbs and lower limbs.
-
All done — Check out your EASI score!

How to interpret your result
The following table illustrates a brief overview of the EASI score ranges:
EASI score | Eczema severity |
---|---|
0.00 | No sign of eczema |
0.01-1 | Almost clear |
1.1-7.0 | Mild |
7.1-21.0 | Moderate |
21.1-50.0 | Severe |
51.1-72.0 | Very severe |
Clear:
There are no visible signs of atopic dermatitis. Your skin appears normal.
Almost Clear:
There are barely any signs of eczema.
Mild:
You have minimal signs of atopic dermatitis. You might present some of the symptoms, but you don't perceive significant thickening or active inflammation.
Moderate:
Your symptoms are noticeable but not severe. Your eczema does not significantly impact daily activities.
Severe:
Your symptoms are intense and cover a significant area of your body.
Very Severe:
Your symptoms are intense and start taking over most of your body areas. You are probably feeling considerable discomfort and impairment.
What does EASI stand for?
The abbreviation EASI refers to the Eczema Area and Severity Index. The EASI score is one of many eczema scales used to clinically assess atopic dermatitis. It helps to clinically assess the intensity of eczema.
What is a 23 EASI score?
A result of 23 falls into the range of severe eczema, while severe atopic dermatitis ranges from 21 to 50. We advise you to consult a physician to verify the diagnosis and establish a treatment plan.
What is lichenification?
Lichenification describes a change in the surface of the skin in which the skin becomes thicker and harder. It is often referred to as leathery. In atopic dermatitis, we can often observe eczema lichenification.
How is EASI calculated?
Follow these steps to calculate the EASI score:
-
Take a look at the first body region (head and neck).
-
Define the area score by estimating what percentage of the area is affected.
-
Look at the intensity of redness, thickening, scratching, and eczema lichenification.
-
Add the numbers from point 3 and receive your body area severity score.
-
To get the score for your body region, multiply the area score by your severity score by its corresponding factor (see the table above).
-
Repeat steps 2-5 for each body region.
-
Add all your body region scores and receive your EASI score.
